Hospital Episode Statistics - HES Maternity Tail (1989-present?)

Although called the “Maternity” Tail, the information requested deals almost exclusively with the baby

Scottish Morbidity Record - Acute Hospital Discharge SMR01 (year-present?)

It seems daft that, despite most U.K. maternity units having a computer system, paper Benchmarking forms still have to be filled in by hand by midwives for someone else to enter onto a different computer program concerning data that has already been entered onto the main maternity computer system.

In any reasonable world, when a Birthrate survey is being done, the maternity computer systems should, on a short term basis, ask the midwife only for any additional data required for Birthrate!!!

During a Birthrate Survey, most of the data required is included in the draft Logical Priority Dataset. Below is a draft proforma for documenting, on paper the additional items not already on the computer. Any good maternity computer system would then automatically ask for these additional data items to be entered on the computer allowing all the Birthrate calculations to be performed automatically.
